goal setting and action plan template

Turn a meaningful wish into a plan that can survive a difficult week.

A completed HeadTidy goal map connecting a calmer morning goal to a measurable outcome, the real obstacle, and a small if-then action plan.
The goal becomes useful when the desired outcome, the obstacle you control, and the first repeatable action are visible together.

What’s included

Make the goal observable

Replace a category such as “get healthier” with evidence you could notice: walk after lunch three times this week, cook two reliable dinners, or book the appointment you have postponed. The outcome branch describes what better looks like; the action branch proves movement.

Plan for the real obstacle

Use the obstacle you control, not a character judgment. “I open work messages after dinner” produces a useful if, then plan. “I lack discipline” does not. Keep the first action small enough to complete on an ordinary day.

Worked example

Wish: feel less rushed in the morning. Outcome: leave home by 8:10 with breakfast eaten. Obstacle: choosing clothes after waking. Plan: if I finish dinner, then I put tomorrow’s clothes beside the bag.
